First-Time Filler in Denver: What to Expect

By Tanna Donalson, PA-COctober 13, 20267 min read

A first filler appointment is mostly conversation. You are numbed for the injecting part, and the face you see afterward is not the one to judge: give it two weeks.

Almost every first-timer asks the same things: will it hurt, will it still look like me, and what if something goes wrong.

What is hyaluronic acid filler, and what is it not?

Hyaluronic acid is a sugar your body already makes. Crosslinking makes it last longer once placed, and your body absorbs it over time. The FDA regulates it as a medical device implant, and is blunt about what that means: having filler injected should be considered a medical procedure, not a cosmetic treatment.

It is not a neurotoxin: Botox relaxes a muscle, filler adds material.

They are also not interchangeable. Juvederm Voluma XC is approved for deep injection for cheek augmentation, to correct age-related volume deficit in the mid-face, in adults over 21. Restylane Lyft with Lidocaine is approved for cheek augmentation and age-related midface contour deficiencies, also over 21. Revanesse Versa is approved for moderate to severe facial wrinkles and folds such as nasolabial folds, in adults 22 or more. Radiesse is approved for those same folds, but it is calcium hydroxylapatite, not hyaluronic acid, which matters later. Skinvive is approved to improve cheek skin smoothness in adults over 21, which is skin quality, not volume.

What happens at a filler consultation?

Before anyone picks up a syringe, you talk. A good consultation starts with what bothers you in the mirror, not a menu of areas.

Health history is what makes this a medical visit rather than a service. The FDA names situations that change the plan: a bleeding disorder, a history of severe allergy or anaphylaxis, active infection at the site, and pregnancy, where safety is unknown.

Only then does a plan get built: which area, which product, how much, and what to expect at two weeks.

What does the appointment feel like?

More ordinary than people expect. The area is cleaned, and topical numbing cream usually goes on first. Many hyaluronic acid fillers also arrive with lidocaine in the syringe: Juvederm Voluma XC is formulated with 0.3% lidocaine. Numbing builds as you go, and the first injection in an area is usually the sharpest moment.

People describe pressure more than pain. You will usually be handed a mirror partway through, and saying you want less is easier than taking it away later.

Needle or cannula?

A needle is sharp and places product where its tip sits. A cannula is a blunt-tipped flexible tube, threaded under the skin from one small entry point to reach a wider area.

Patients often ask for a cannula, having read it is the safe option. The clinical guideline for managing filler emergencies says vascular complications can occur using either one, and cites a case series in which most severe complications had involved cannulas. What a cannula does offer is fewer entry points. Which one your injector picks is a judgment about area, product and depth, not a safety switch.

How long do swelling and bruising last?

You see a change immediately. You also see swelling, and on day one the two look alike.

The FDA study behind Juvederm Voluma XC tracked this day by day, and nearly everyone logged something at the treatment site. Swelling most often lasted one to three days, bruising four to seven. For responses rated moderate or severe, the median time to complete resolution was six days. Lumps and firmness took longest: about a third of people still reported lumps past two weeks.

Two weeks is the standard point to judge a result or add more. The FDA notes some side effects appear weeks, months, or years later, so call rather than wonder.

The FDA puts hyaluronic acid at roughly 6 to 12 months and calcium hydroxylapatite at roughly 18. In the Voluma study 85% were still responders at 12 months, in people given a mean of 6.8 mL across an initial treatment and a touch-up, far more than a first visit.

How do you keep from looking overdone?

Mostly restraint.

Start with less than you think you need. Adding more later is a small visit; taking away is a second procedure.

Treat one area at a time, or you cannot tell which one did the work.

Learn the difference between structure and volume. The labels show it: Voluma goes deep, against bone, for cheek structure. Versa goes into the dermis to soften a fold. Skinvive goes shallower still, for smoothness. Three depths, three jobs. The overfilled look usually comes from chasing volume in a face that needed structure, or from topping up the same area year after year.

Hyaluronic acid filler can also be softened or removed with hyaluronidase, and that reassurance has a limit. Hylenex is FDA-approved to help other injected medicines spread and absorb, so using it on cosmetic filler is an off-label use, and a deliberate one. It works on hyaluronic acid, so Radiesse cannot be dissolved this way. Our post on dissolving filler covers it.

What are the real risks?

The common ones: bruising, redness, swelling, pain, tenderness, itching, a rash. Expected, temporary, not a sign anything went wrong.

The FDA names the serious one plainly: the most concerning risk associated with the use of dermal fillers is unintentional injection into a blood vessel. Reported complications include necrosis, meaning death of tissue, vision abnormalities including blindness, and stroke, and the FDA notes they can be serious and may be permanent.

This is rare: the clinical guideline written for injectors calls it a relatively rare occurrence. It is also why who holds the syringe matters, because catching it early and treating it fast changes the outcome. The treatment is prompt hyaluronidase.

So learn the warning signs before you leave. Pain out of proportion to what you were told to expect, pain away from the injection site, skin that turns pale, dusky or mottled purple, and any change in vision are not normal, and can appear hours or days later. Our guide to how to choose a medical spa in Denver covers what else to ask.

Frequently asked questions

How long before I can judge my result?

About two weeks. In the Voluma study about a third still reported lumps past the two-week mark, so an earlier verdict is a verdict on swelling.

Will people be able to tell?

The visible part of a first appointment is usually the first week's bruising and swelling, not the filler. A small amount in one area keeps a result quiet.

Can filler be undone?

Hyaluronic acid filler can be softened or removed with hyaluronidase, an off-label use of an FDA-approved enzyme. Radiesse is calcium-based and cannot.

What if something feels wrong after I get home?

Call rather than waiting. Severe or spreading pain, skin that goes pale or mottled purple, or any vision change needs to be seen the same day.

What to expect at Redbud

Your first filler visit at Redbud starts with a conversation and a mirror. Your injector is a licensed medical provider who looks at your face at rest and while it moves, and says plainly if the answer is a smaller amount, a different treatment, or nothing today.

We treat conservatively on purpose, and would rather do less at a first visit than more. We keep a dissolver on hand and are prepared for the rare emergency, because this is a medical procedure. Our guide to lip filler pricing in Denver explains what sits behind a quote, and restoring facial volume after a GLP-1 covers filler after weight loss.
